Forms that have "TBD" for golfer information will not be accepted
DOOR Prizes:
WE NEED RAFFLE PRIZES!! Please consider donating a prize such as small electronics, golf accessories or gift cards for the raffle at the evening event. The actual prize can either be delivered to our office prior to Wednesday, July 17th or brought to the course on the day of the tournament. PLease be sure to attach your business card to the prize so that we can recognize your company during the prize raffle. THANK YOU!
